Ringkasan Ungkup Pekerjaan atau Spesifikasi Barang / Scope of Work or Specification of Goods Summary : Kontraktor harus menyediakan 4 (empat) Unit Small Craft untuk Paket No. 3: C1511371 4 units small craft (LCT / Landing Craft Tank):

a. 2 units Charter Time (LCT approx 35 m LOA no bow thruster & LCT Approx 53 m LOA, min 210m2,1000 BHP Class)

b. 2 Units Call Out (LCT approx 53 m LOA no bow thruster, 1000 BHP Class 8i LCT approx 45 m LOA, 1000 BHP Class)

Kapal tersebut diperlukan untuk mendukung operasi Perusahaan di lepas pantai Kalimantan Timur termasuk transportasi dan supply barang & peralatan, serta angkutan untuk limbah B-3. / Contractor shall provide 4 (four) units Small Craft in-' package No. 3 as per above description to support Company In off-shore operations East Kalimantan including transportation and supply for material & equipment and hazardous waste.

A. Persyaratan Pendaftaran / Registration Requirement

Jenis Pengadaan / Type of Procurement : Other Services (Jasa Lainnya)

Bidang Usaha / Business Section : Jasa Angkutan / Transportation Services

Sub-bidang Usaha / Business Sub section : Peralatan Angkut Perairan antara lain: Barge, Personnel Boat

Batasan Minimal TKDN / Minimum Threshold of Domestic Content : 90%

Ketentuan-ketentuan untuk Konsorsium / Provisions for Consortium:

1. Pemuka Konsorsium / Leodfirm :

Dalam hal Konsorsium, Perusahaan Dalam Negeri harus bertindak sebagai Pemuka Konsorsium (Leodfirm)/ In the event of Consortium, Domestic Company must oct as Leadfirm of Consortium.

2. Keanggotaan Perusahaan Asing / Foreign Company membership

Konsorsium tidak dapat beranggotakan Perusahaan Asing / Consortium can not be composed of Foreign Company.

B. Dokumen Pendaftaran / Registration Document

1. Surat Pengantar Pendaftaran Tender/ Cover Letter of Tender Registration (FORM/P4/13/REV.002).

2. Sertifikat Pengganti Dokumen Administrasi (SPDA) dari sistem Centralized Integrated Vendor Database (CIVD).

Bag! Calon Peserta yang mendaftar sebagai Konsorsium, Persyaratan ini berlaku bagi semua anggota konsorsium/This " requirement is applicable for all consortium members in the event that Bidder Candidate registers as Consortium.

Jika anggota konsorsium bukan berbadan hukum Indonesia (Perusahaan Asing), maka SPDA dalam CIVD tidak dipersyaratkan, namun tetap menyampaikan dokumen-dokumen yang dipersyaratkan, antara lain: Akta Pendirian dan Sertifikat Domisili / In the event the consortium member is not Incorporated under Indonesia law (Foreign Company), SPDA CIVD is not required, but It has to submit required documents, i.e.: Article of Incorporation and Certificate of Demtofe.

3. Salinan surat izin usaha pada bidang/sub-bidang usahanya yang masih berlaku sesuai paket Tender, dalam hal ini adalah Surat Izin Usaha Perusahaan Angkutan Laut (SIUPAL) atau Surat Izin Operasi Perusahaan Angkutan laut Khusus (SIOPSUS) yang masih berlaku yang dikeluarkan oleh Departemen Perhubungan Laut Indonesia /

Copy of valid business license on similar Business Section/Business Sub-section with Tender package means the valid Sea Transportation Permit Letter (SIUPAL) or Special Operational Sea Transportation Permit Letter (SIOPSUS) which is issued by Indonesia Sea Communication Dept.

4. Salinan sertifikat TKDN yang diterbitkan oleh instansi pemerintah yang membawahi bidang perindustrian atau dokumen kesanggupan pemenuhan komitmen TKDN (FORM P4/20/REV.000)/ Copy of Domestic Content certificate issued by government agency who oversees industry sector or document on capability to the commitment fulfilment of Domestic Content (FORM P4/20/REV.000).

Pengumuman ini mengacu pada Pedoman Tata Kerja SKK Migas No. PTK-007/SKKO0000/2015/S0 Buku Kesatu Revisi 02 dan Buku Kedua Revlsl 03 (PTK 007 Revisi Tahun 2015) / This announcement is subject to the provisions under SKK Migas Work Procedure "No. PTK-O07/SKKO0000/2015/S0 Buku Kesatu Revisi 02 dan Buku Kedua Revisi 03 (PTK 007 Revisi Tahun 2015)".

2. Ketentuan lain terkait prosedur pendaftaran akan mengacu kepada PROSEDUR PENDAFTARAN TENDER yang dapat diunduh meialui laman http://www.chevronindonesia.com menu pengadaan barang & jasa ; prosedur pendaftaran tender / Other provisions in relation to the registration procedure shall refer to TENDER REGISTRATION PROCEDURE which can be downloaded from the website http://www.chevronindonesia.com menu procurement - tender registration procedure.

3. Hanya Calon Peserta yang telah memenuhi persyaratan pendaftaran yang dapat mengikuti proses kualifikasi. / Only Bidder Candidate who have met the registration requirements may participate in qualification process.

4. Pendaftaran di luar waktu dan tempat pendaftaran di atas tidak dapat diterima. / Registration beyond the above designated time and place is unacceptable.
